Zusammenfassung
Die Therapie der idiopathischen peripheren fazialen Paresen (Bellschen Lähmung) wird
zunehmend unter dem Blickwinkel einer ambulanten Versorgung zu sehen sein. Wir legen
eine Zusammenstellung vor mit Bewertung der Behandlungsmaßnahmen unter Effektivitäts-
und Kostenaspekten. Eine wichtige Stellung nimmt die Verhütung von Sekundärschäden
ein. Unter den ätiologisch ausgerichteten Therapien scheint nur die mit Kortikosteroiden
begründet. Zur Verbesserung der Regeneration nach axonaler Schädigung haben sich Bewegungsübungen
vor dem Spiegel bewährt. Es verwundert, dass Therapieformen ohne ausreichenden Wirksamkeitsnachweis
immer noch empfohlen werden. Zur Einschätzung von Studienergebnissen ist zu berücksichtigen,
dass Patientenkollektive unter 190 - 200 nicht die erforderliche Signifikanz erwarten
lassen, da viele Kollektive heterogen sind.
Treatment of the Idiopathic Peripheral Facial Paresis (Bell's Palsy)
The actual therapeutic concepts of Bell's palsy are discussed regarding efficacy and
costs. There are three main fields that require attention. 1. Prohibition of secondary
lesions of the eye due to lagopthalmos is of major practical relevance and needs careful
management. 2. For treatment of the nerve lesion itself only corticosteriodes can
be recommended so far. Data on antiviral and rheologic medication are sparse and up
to now do not meet evidence based criteria. 3. The best concept for improving regeneration
after axonal damage is grimacing utilizing visual control. Almost all investigations
on the therapeutic regime of Bell's palsy published in the last 20 years fail to meet
the estimated number of 190 - 200 patients providing significant results.
